If you have one of our puppies and would like to have your puppy's microchip transferred to you, please complete the following information and upload a copy of proof of spay/neuter using this for.. Please be sure the chip number is on the spay/neuter paperwork.​

You will receive an email confirming chip registration, usually within a few days.
